Student Paperwork
In order for your student to have the testing accommodations from their IEP or 504Plan during the SAT, PSAT, or AP exams, you must give Maker consent to share those accommodations with the College Board. To do that, email [email protected] and include your child's name in the email. Only for 10th, 11th, and 12th graders.
Working papers requirements
Bring to Sruthi in office 441
- An application with Section 1 completed and signed by a parent (click button below)
- An original version of a:
- New York State doctor's note (stating the student is "fit for employment")
- Certificate of physical fitness completed by a NY State physician
- Schooling record:
- Registered NYC public school students need their school ID
